Val’s helps eligible individuals and families with approved personal-care and in-home support needs so they can remain supported in their homes and communities.

SERVICES OVERVIEW

The right support depends on the individual.

Services depend on assessed needs, the program involved, authorization when required, the approved plan of care, and availability. Val’s can explain how to begin but does not independently determine eligibility.

PERSONAL CARE SERVICES

Help with activities of daily living.

Personal Care Services can provide hands-on assistance with routine activities that may be difficult for a person to complete independently. The goal is practical support in the home based on the person’s assessed needs.

Services provided depend on assessed needs, authorization when required, and the individual plan of care. Every task listed will not apply to every person.

COMMUNITY ALTERNATIVES PROGRAMS

CAP support, explained plainly.

CAP/DA and CAP/C are separate Medicaid-related programs. Approved services vary by program and by the individual care plan.

CAP/DA

Community Alternatives Program for Disabled Adults

CAP/DA supports eligible adults with qualifying needs who may receive approved services in their home or community.

Eligibility is determined through the appropriate program process—not by Val’s alone. Services depend on authorization, the approved care plan, and agency availability.

Community Alternatives Program for Children

CAP/C supports eligible children with qualifying needs who may receive approved services and support in their home or community.

The appropriate program process determines eligibility. Val’s does not approve program participation, and services depend on authorization, the approved care plan, and agency availability.

PERSONAL CARE SERVICES UNDER 21

Approved personal care for eligible individuals under age 21.

Val’s may provide Personal Care Services Under 21 based on the applicable assessment, authorization, and plan of care. The program’s appropriate process—not Val’s independently—determines eligibility and approval.

RESPITE SUPPORT

Temporary support for caregiver relief.

Respite can give a family caregiver a period of relief while an eligible individual receives supervision and approved support. Arrangements depend on the applicable service plan and staff availability.

COMPANION / SITTER SUPPORT

Non-medical support for everyday routines.

Companion or sitter support may include companionship, supervision, safety awareness, routine household support, and help staying engaged in everyday activities. It is not clinical care.

EVERYDAY HOME SUPPORT

Common ways support may look at home.

These examples describe non-medical tasks that may be provided when appropriate and included in the approved service arrangement.

Assistance with routine personal hygiene according to assessed needs.

Dressing and grooming

Support with getting dressed and completing grooming routines.

Toileting

Personal assistance provided with respect for privacy and dignity.

Eating assistance

Help with routine eating needs when included in the plan of care.

Meal preparation

Preparing routine meals that fit the approved service arrangement.

Medication reminders

Reminders only—not medication administration or medication management.

Mobility and transfers

Appropriate assistance with movement and transfers when included in the plan.

Daily routine support

Help with everyday activities and light routine support around the home.

UNDERSTANDING THE TYPE OF CARE WE PROVIDE

Routine personal care and in-home support.

Val’s primarily provides non-emergency personal care and in-home support. These services should not be understood as hospital-level care, emergency medical services, medical diagnosis, medication administration, or an unverified skilled nursing service. Val’s also does not independently determine Medicaid eligibility.

Requirements vary by service and program. These steps are a general guide and do not promise eligibility, case acceptance, immediate staffing, or a start date.

    Assessment, eligibility, authorization, or referral steps occur through the appropriate process for the program involved.

    Confirm whether Val’s can accept the case

    If services are authorized, Val’s reviews the request, approved needs, service area, and staffing availability.

    Establish the service arrangement

    When the case can move forward, intake and service planning are completed according to the approved plan.
